PERSONAL INFORMATION:
	
	1.Enter your full name and address as it appears on the check.
	  If there is more than one name on the check, please only enter
	  your own. 
	2.Enter your phone number with spaces or dashes. (Example: 703-555-1212)

PAYMENT INFORMATION:
	
	1.Enter the numbers at the bottom of the check from left to right.
	  Exclude the check number, which will sometimes be listed in front,
          in the middle or at the end of the numbers at the bottom of the check.
          You can identify the check number by matching it up with what number is
          in the upper right hand corner of your check. Do not enter this in the
          "Numbers at bottom of Check" area. 
	2.Enter check number found in the upper right hand corner, enter into the
	  "CHECK" field.
	3.Enter the dollar amount. Enter using a decimal point but no dollar sign.
          (Example: 150.00)
	4.Lyons, Doughty & Veldhuis file Number should be entered in the proper format.
          (Example: 04-12345-0)  This can be found on any correspondence you have received
          from us at the bottom left of the page.
	5.When information is complete, Click "SUBMIT".  
	6.Verify payment information. If information is correct, click "SUBMIT".  If any
          of the information is incorrect, Click "CORRECT" and make any necessary adjustments.
	7.Your transaction has been submitted. Please select "RECEIPT" to print yourself a
          copy of the payment record. Then select "EXIT" to securely exit the payment screen.
